Add cocoa powder and instant coffee or espresso powder (if using) to the blender. Add vanilla ice cream and pour in the milk or chilled coffee.

Blend until the mixture is completely smooth and pourable, scraping down the sides if necessary.

Pour the milkshake into a chilled glass. If desired, drizzle fudge sauce along the interior of the glass before filling.

Top with whipped cream and garnish with chocolate shavings. Serve immediately.
